Transferring Students

NJ Administrative Code citation 8:57-4.1 to 8:57-4.20 mandates that students must meet specific criteria when entering a New Jersey school. Please see below to learn about these requirements. The paperwork must be submitted at the times stated.

Student Information Sheet - this form MUST be submitted to the health office when your child begins school.
Entrance physical examination - this form must be based on an exam performed within one year of the date that your child registers for school. A Universal Health Form will be accepted.
New Jersey Minimum Immunization Requirements - if your child is transferring from a school within New Jersey, immunization documentation MUST be submitted in order for your child to begin school. If they are transferring from out-of-state or out-of-country, you will be allowed 30 days to submit this information.

Medications - if your child needs emergency medication to be kept at school, please see the district medication policy on the 'Medications in School' tab.
No medication will be accepted without a correctly completed medication form from your child's doctor.
